Title: One-stage restitution of nasal and lip deformity associated with a bilateral cleft lip
Abstract: Objective:To explore the surgical method of correcting cleft lip and nasal deformity at one-stage in restitution of congenital bilateral complete cleft lip in order to get more satisfactory cosmetic operative effect and decrease the difficulties of repairing secondary nasal deformity.Methods:The chief design principle of cleft lip restitution was keeping the length of prolabium,and at the same time the philtrum and nostrils width were adjusted by little triangle besides columella nasi.The orbicularis oris muscle was dissected and replaced. During operation,nasal deformity was rectified by remodeling position of greater alar cartilage,recover rhino-dome and lengthen columella nasi through the operative incision of bilateral nostril verge.Results:Fifteen patients were treated with this surgical method. Their ages were from six months to twelve months.Primary healing of the incision was achieved and the blood supply of anterior lip was sufficient.The followed-up was as long as three years.20 of these cases showed satisfactory results with desirable height of lip,regularity of red labial margin,nice Cupid's bow,turgor vitalis of vermilion of the lip,continual orbicularis oris muscle when lip exercising.Their collapse ala nasi significantly improved;the double nostrils were similar,and the columella nasi were lengthened.5 of these cases achieved satisfactory effect of repair of cleft lip but their nasal deformity were only partly improved.Conclusion:The good operative effects could be achieved when cleft lip was repaired and nasal deformity was corrected at one-stage in bilateral complete cleft lip.Therefore,this surgical technique is recommended to the bilateral cleft lip.
