Title: Treatment of Severe Closed Heads Injuries with Bilateral Balance Decompressive Craniectomy
Abstract: Objective To explore the curative effect of the active bilateral balance decompressive craniectomy on acute severe closed head injuries. Methods Of 256 patients with acute severe closed head injuries undergoing bilateral decompressive craniectomy, 120 were treated by active bilateral decompressive craniectomy completed by an operation (group A) and 136 by passive bilateral decompressive craniectomy completed by early and late operations in twice (group B). The prognosis and complications were compared between both the groups. Results The mortality (19.17%) in group A was significantly lower than that (29.41%) in group B (P0.01). The percent of patients with good prognosis (29.17%) was significantly higher in group A than that (18.38%) in group B (P0.01). Conclusions The active bilateral decompressive craniectomy can prevent the delayed intracranial hematomas, stabilize the bilateral intracranial composition, reduce the secondary injuries to brain due to the brainstem flickering, and significantly reduce the mortality and disability, so it is a simple and effective method for the surgical management of severe closed head injuries.
