Title: Etiological analysis of urinary tract infections in cervical cancer patients after radical hysterectomy
Abstract: OBJECTIVE To analyze the etiological characteristics of urinary tract infections in the cervical cancer patients after radical hysterectomy and observe the molecular biochemical parameters so as to provide guidance for the diagnosis and treatment for postoperative urinary tract infections.METHODS A total of 367cases of cervical cancer patients,who underwent the radical hysterectomy in the hospital from Jan 2010to Feb 2013,were enrolled in the study,then the clinical data of the subjects were retrospectively analyzed,the patients with postoperative urinary tract infections were assigned as the infection group,while the patients without urinary tract infections were set as the non-infection group,the identification of the pathogens causing infections,the drug susceptibility testing,and the detection of biochemical indicators were performed,the laboratory operations such as culture were carried out by strictly according to the National Clinical Laboratory Operation Regulations,and the statistical analysis was performed with the use of SPSS16.0software.RESULTS Of the 367patients,the urinary tract infections occurred in 133cases with the infection rate of 36.24%,among which 55were cultured positive for submitted urine specimens with the positive rate of 41.35%.A total of 108strains of pathogens have been isolated,including 65(60.19%)strains of gram-negative bacteria,28(25.92%)strains of gram-positive bacteria, and 15(13.89%)strains of fungi.The drug resistance rates of the Escherichia coli to ceftazidime,cefazolin, ampicillin,ampicillin-sulbactam,piperacillin,piperacillin-tazobactam,and ciprofloxacin were 62.50%,94.59%, 97.30%,74.29%,89.19%,75.68%,and 57.58%,respectively;the drug resistance rates of the Klebsiella pneumoniae to ampicillin,ampicillin-sulbactam,and piperacillin were 85.71%,64.29%,and 84.62%,respectively. The difference in the level of serum FSH,LH or E2between the patients with urinary tract infections and the patients without infections was not significant.CONCLUSIONIf the clinical laboratory condition permits,it is necessary to perform the urine culture for pathogens and the drug susceptibility testing for the patients with suspected urinary tract infections after radical hysterectomy and choose sensitive antibiotics based on the result of drug susceptibility testing so as to improve the clinical therapeutic effect.
