Title: Clinical efficacy of early BIPAP of noninvasive ventilation in the treatment of COPD complicated with type IIrespiratory failure
Abstract: Objective To investigate the effect of early treatment of two-level positive airway pressure noninvasive ventilation (BIPAP) in the treatment of chronic obstructive pulmonary disease (COPD) complicated with type Ⅱ respiratory failure. Methods Sixty patients with COPD complicated with type Ⅱ respiratory failure were randomly divided into treatment group and control group. Control group received routine therapy: low concentration of oxygen, anti-infective, anti-asthmatic, expectorant therapy. Treatment group received BIPAP therapy additionally. Between two groups, arterial blood gas analysis indexes before and after treatment, fatality rate and length of stay were compared. Results After treatment, in two groups pH, PaO2and SaO2 were increased, PaCO2was decreased(P0.05). The levels of pH, PaO2, SaO2 were increased and the level of PaCO2 was decreased more significantly in treatment group than those in control group (P0.05). The decrease of fatalify rate, length of stay was significantly more in treatment group than those in control group (P0.05). Conclusions There are significant curative effect of early treatment of BIPAP in COPD with type Ⅱ respiratory failure.
