Title: Cystic Teratoma-Provoked Peritonitis After Pregnancy Termination
Abstract: Background: Cystic teratoma is an unusual cause of postabortal pelvic peritonitis. Case: A healthy 22-year-old woman underwent outpatient first-trimester vacuum pregnancy termination, followed by a 3-day course of oral doxycydine. On the thirteenth day after pregnancy termination, she presented with fever, an acute abdomen, and pelvic pain. The woman underwent re-aspiration of her uterus, which yielded scanty tissue. She then underwent pelvic ultrasound and computerized tomogram, both of which revealed a posterior pelvic mass, presumed to be an abscess. After 5 days of broad-spectrum antibiotics had failed to alleviate her fever and pain, culdocentesis was performed yielding 30 cm3 of pus, hair, and fat. The woman underwent laparotomy, which revealed a 10 × 8 × 5 cm multiloculated, infected benign cystic teratoma with extensive capsular adhesions. Conclusion: If postoperative infection is to develop after pregnancy termination, signs will usually appear within the first week. Onset of pelvic peritonitis more than I week after uneventful conclusion of a pregnancy event should raise suspicion of either non-gynaecologic causes or, as in this case, a pelvic neoplasm. Contexte: Le tératome kystique est une cause inhabituelle de pelvipéritonite post-abortum. Cas: Une femme en santé de 22 ans a subi une interruption de grossesse (premier trimestre) par aspiration en consultation externe, suivie d’un traitement de trois jours à la doxycycline (par voie orale). Treize jours après l’interruption de grossesse, elle présentait de la fièvre, un abdomen aigu et des douleurs pelviennes. La patiente a de nouveau subi une aspiration de l’utérus, laquelle n’a permis que l’obtention d’une faible quantité de tissus. Elle s’est alors soumise à une échographie et à une tomographic par ordinateur pelviennes, lesquelles ont toutes deux révélé une masse pelvienne postérieure (on a alors présumé qu’il s’agissait d’un abcès). Après l’échec d’un traitement de cinq jours aux antibiotiques à large spectre visant l’atténuation de la fièvre et de la douleur, une culdocentèse a été effectuée (on en a tiré 30 cm3 de pus, de poils et de tissu adipeux). La patiente a subi une laparotomie, laquelle a révélé un tératome kystique, bénin infecté, de 10 × 8 × 5 cm, multiloculaire et accompagné d’adhérences capsulaires étendues. Conclusion: Lorsqu’une infection postopératoire est engendrée à la suite d’une interruption de grossesse, les symptômes qui y sont associés apparaissent habituellement au cours de la première semaine. La survenue d’une pelvipéritonite, plus d’une semaine à la suite d’une interruption de grossesse sans complication, devrait laisser soupçonner la présence de causes non gynécologiques ou, comme dans le présent cas, d’un néoplasme pelvien.
