Abstract: A cne vulgaris is a common dermatological pathology whose causes, although extensively studied in the last decade, have not been fully elucidated. The involvement of Propionibacterium acnes in the pathogenesis of the disease, by the onset and maintenance of a follicular inflammatory status, has been disputed over the years. Paradoxical evidence suggests both its role as a skin commensal in patients without lesions and as a skin pathogen, that triggers a sustained follicular inflammatory response in patients diagnosed with acne vulgaris. The presence of Propionibacterium acnes communities with different locations within the pilosebaceous follicle, with variable virulence patterns and organized in complex biofilm structures might represent the first arguments in favour of a commensal/pathogen switch. We aim to review the latest findings on the involvement of Propionibacterium acnes in acne pathogenesis. Acneea vulgara este o patologie dermatologica frecventa ale carei cauze, deși studiate intens in ultimul deceniu, nu au fost complet elucidate. Implicarea Propionibacterium acnes in patogeneza acestei afecțiuni, prin declanșarea și menținerea unui proces inflamator cronic la nivel folicular, a fost indelung disputata in ultimii ani. In mod paradoxal, rezultatele studiilor au evidențiat atât rolul sau de comensal cutanat la pacienții fara leziuni, cât și acela de patogen capabil sa declanșeze un raspuns inflamator folicular, la pacienții diagnosticați cu acnee. Existența unor comunitați de Propionibacterium acnes dispuse diferit la nivel folicular, cu profiluri de virulența variabile, precum și organizarea in structuri complexe de tip biofilm pot reprezenta primele argumente in favoarea unei transformari comensal/patogen.
