Title: Electrophysiological Basis of ECG Characteristics of Torsades de Pointes in Long QT Syndrome
Abstract: In this article, the electrophysiologic basis of the various electrocardiogram characteristics of torsades de pointes (TdP) arrhythmias in long QT syndrome (LQTS) is presented. The in vivo electrophysiologic mechanism of TdP in LQTS is described using, as a paradigm, the anthopleurin-A canine model. In LQTS, prolonged repolarization is associated with increased spatial dispersion of repolarization. Prolongation of repolarization also acts as a primary step for the generation of early afterdepolarizations. An electrophysiologic basis of the characteristic twisting QRS configuration of TdP is discussed. Also explained is the mechanism of the short-long cardiac sequence.
