Title: Food Safety Assurance Systems: Investigation of Incidents in Industry
Abstract: This article describes the main elements of incident investigation from the perspective of a food manufacturer. This includes a description of the key elements that need to be considered, including 'signals' indicating that an incident has or may have occurred, key steps in incident investigation, gathering of relevant information, and identification of direct and underlying (root) cause(s). The situations considered focus mostly on incidents caused by biological hazards that may be present in products arising from failures in raw materials sourcing/supply, manufacturing, transport, and distribution. The main purpose of this article is to provide guidance on the principles that should be applied in incident investigation that will aid in the preparedness for such incidents should they occur.
