Title: 412 IMPROVED DIAGNOSTIC ACCURACY OF TRANSIENT ELASTOGRAPHY FOR CIRRHOSIS USING HISTOLOGY PLUS CLINICS AS GOLD STANDARD
Abstract: Background and Aim: Although histopathological analysis of liver biopsy is still the gold standard for the evaluation of disease progression in chronic hepatitis, methods evaluating liver fibrosis have made a great progression by using noninvasive tools.Especially, liver stiffness measurement by transient elastography (FS) has established a position as the center of these tools.We present Real time tissue elastography (RTE) for the visual assessment of liver stiffness.RTE has also been evaluated in the breast cancer, thyroid grand, prostate and pancreas.Patients and Methods: RTE (EUB-8500, EUP-L52 Linear probe; 3-7 MHz, Hitachi Medical Corporation) based on the Combined Autocorrelation Method to rapidly calculate the relative hardness of tissue from the degree of tissue distortion and displayed this information as color image at real time.For quantitative analysis the values of tissue elasticity from RTE image obtained by a novel software (Elasto_ver1.5.1.Hitachi Medical Corporation).107 patients with chronic hepatitis C were assessed histological fibrosis stage and received FS (Fibroscan, Echosens) and detailed blood examinations within two days after RTE procedure.Results: Six (6%) patients were classified as F0, 52 (48%) as F1, 18 (17%) as F2, 14 (13%) as F3, and 17 (16%) as F4 (cirrhosis).Elasto_ver1.5.1 was selected four individual parameters as the value of tissue elasticity.Mean and Standard deviation (SD) were described from a histogram.Area and Complexity were described from a binary image.Simple regression analyses indicated that Mean, SD, Area, and Complexity were significantly correlated with liver stiffness (Stiffness) obtained by FS. (Mean: r= -0.593, p < 0.001; SD: r= 0.449, p < 0.001; Area: r= 0.612, p < 0.001; Complexity: r= 0.549, p < 0.001).The area under the receiving operating characteristic curve (AUC) for stage no significant fibrosis (F0-1) were 0.93, 0.85, 0.91, 0.85 and 0.91 for Mean, SD, Area, Complexity and Stiffness respectively.The AUC for cirrhosis (F4) were 0.93, 0.86, 0.93, 0.96 and 0.92 for Mean, SD, Area, Complexity and Stiffness respectively. Conclusion:The performance of RTE is favorable compared with FS for the presence of significant liver fibrosis in patients with chronic hepatitis C.
