Title: Kontrastmittelverstärkte MR-Angiographie zur Differenzierung zwischen perigastrischen und submukösen Fundusvarizen des Magens
Abstract: Fragestellung: Das Ziel dieser Studie war es, die kontrastmittelverstärkte MR-Angiographie bei der Diagnose von Fundusvarizen des Magens, insbesondere bei der Differenzierung von perigastrischen und submukösen Fundusvarizen mit dem endoskopischen Ultraschall (EUS) zu vergleichen. Methoden: Neunzehn Patienten mit klinischem Verdacht auf Fundusvarizen wurden prospektiv in die Studie eingeschlossen und innerhalb einer Woche mittels kontrastmittelverstärkter MR-Angiographie und EUS untersucht. Die Übereinstimmungen zwischen MR-Angiographie (Radiologe 1 und 2) und EUS (1 Gastroenterologe) bei der diagnostischen Sicherheit bezüglich der Diagnose perigastrischer und submuköser Fundusvarizen sowie bei der Größen- und Lokalisationsbestimmung submuköser Fundusvarizen wurden untersucht. Ergebnisse: Sowohl bezüglich der Diagnose perigastrischer (19/19 Patienten) als auch submuköser Fundusvarizen (14/19 Patienten) stimmten beide Radiologen und der Gastroenterologe bei allen Patienten überein. Bezüglich Quantifizierung der Durchmesser der submukösen Fundusvarizen und der Lokalisationen innerhalb des Fundus waren die Übereinstimmungen zwischen den Radiologen gut respektive exzellent (κ = 0,76 respektive κ = 1,0). MR-Angiographie und EUS stimmten bei der Größen- und Lokalisationsbestimmung submuköser Fundusvarizen jeweils bei 12/14 Patienten (86 %) überein. Schlussfolgerung: Die kontrastmittelverstärkte MR-Angiographie erlaubt im Vergleich zum EUS eine vergleichbar gute Diagnose und Charakterisierung von Fundusvarizen des Magens.
