Title: Lymph node tuberculosis: A comparison of various methods of treatment
Abstract: The treatment of lymph node tuberculosis has been studied in 108 patients. Chemotherapy consisted of 18 months' isoniazid with either rifampicin or ethambutol, plus an initial supplement of streptomycin. These 2 regimens were randomly allocated to 19 patients who had previously undergone excision of the lymph nodes, 56 patients who had undergone biopsy and 33 patients who had not had any surgical procedures. The majority of patients were of Indian or Pakistani origin (80%). Histological evidence of tuberculosis was obtained in 64% of the 108 patients and M. tuberculosis was cultured in 30%. No other mycobacteria were grown. Progress during treatment was uneventful in 65% of patients. Fresh nodes appeared during treatment in 12%, existing nodes enlarged in 13% and fluctuation developed in 11% of patients. Discharge and/or sinus formation was infrequent (7%), as was breakdown of a surgical scar (4%). Excision or aspiration after the start of chemotherapy was performed in 19% of patients. In the period up to the end of chemotherapy no difference emerged between the patients who received rifampicin with isoniazid and those who received ethambutol with isoniazid. Satisfactory results were obtained in 98% of the patients by the end of treatment although 13% still had slight node enlargement. The place of surgery is discussed. Follow-up will continue for 18 months after the end of chemotherapy. Le traitement de la tuberculose des ganglions a étéétudiée chez 108 malades. La chimiothérapie a consisté en 18 mois d'isoniazide en association avec soit de la rifampicine, soit de l'éthambutol, avec un supplément initial de streptomycine. Ces 2 régimes ont été attribués de fatçon aléatoire à 19 malades qui avaient antérieurement subi l'excision des ganglions lymphatiques, à 56 malades qui avaient subi une biopsie et à 33 malades à qui aucune intervention chirurgicale n'avait ét/'e appliquée. La majorité des malades était d'origine indienne ou pakistanaise. La preuve histologique de la tuberculose a été obtenue chez 64% des 108 malades et le Mycobacterium tuberculosis a été cultivé chez 30% d'entre eux. Aucune autre mycobactérie n'a poussé. L'excision sous traitement a été sans histoire chez 65% des malades. Chez 12% d'entre eux, de nouveaux ganglions sont apparus, chez 13% les ganglions existants ont augmenté de volume et chez 11% les ganglions sont devenus fluctuants. L'écoulement et ou la formation d'une fistule a été rare (7%) de même que l'ouverture d'une cicatrice de chirurgie (4%). L'excision ou l'aspiration après le début de chimiothérapie a été réalisée chez 19% des malades. Au cours de la période qui s'est écoulée jusqu'à la fin de la chimiothérapie, aucune différence n'a été enregistrée entre les malades qui recevaient de la rifampicine avec de l'isoniazide et ceux qui recevaient de l'éthambutol avec de l'isoniazide. Des résultats satisfaisants ont été obtenus à la fin du traitement chez 98% des malades bien que 13% d'entre eux présentait encore à ce moment là une légr̀e augmentation du volume du ganglion. Las ulceraciones o fístulas fueron infrecuentes (7%) e igualmente la fistulación de cicatrices quirúrgicas (4%). Se practicó extirpación o aspiración después de iniciada la quimioterapia en 19% de los pacientes. Durante el período de tratamiento no se observó ninguna diferencia entre los enfermos que recibieron rifampicina con isoniacida y aquellos que recibieron etambutol con isoniacida. Se obtuvieron resultados satisfactorios en 98% de los pacientes al final del tratamiento aunque 13% tenian aún un pequeño aumento ganglionar. El rol de la cirugía es discutido. El seguimiento continuará durante 18 meses después de finalizada la quimioterapia.
