Title: T-Zellreaktivität und HLA-DR-Bindung des Latexallergens Hev b 1
Abstract: Hev b 1 ist das erste molekularbiologisch charakterisierte Latexallergen. Es hat sowohl für Patienten mit Spina bifida als auch für Beschäftigte im Gesundheitswesen allergenes Potenzial. Im Rahmen der vorliegenden Untersuchungen verglichen wir die zellulären Antworten in den zwei Kollektiven latexallergischer Patienten (Spina bifida-Patienten versus Beschäftigte im Gesundheitswesen) gegenüber dem nativen Hev b 1 und überprüften die Reaktivität von rekombinant hergestelltem Hev b 1. Letzteres ist ein Fusionsprotein bestehend aus Maltosebindeprotein und rHev b 1 (MBP-rHev b 1). Hev b 1 induzierte dosisabhängig in peripheren mononukleären Blutzellen (PBMC) von mehr als 50 % der Latexallergiker (n = 20) aus dem Gesundheitswesen und von 92 % der latexallergischen Patienten mit Spina bifida (n = 13) eine signifikante Proliferationsantwort unabhängig von der Höhe des Hev b 1-spezifischen IgEs im Serum. MBP-rHev b 1 erwies sich ebenso wie natives Hev b 1 in den meisten Fällen als guter Induktor der Proliferationsantwort. Das T-Zellepitop-Mapping mit neun überlappenden Peptiden ergab, dass die gesamte Proteinsequenz immunogen ist, allerdings mit unterschiedlichen Intensitäten. 75 % der PBMC reagierten mit dem Peptid 91 - 109. Unter Verwendung stringenter Bedingungen für die Aminosäurezusammensetzung der Ankerpositionen P1, P4, P6 und P9 wurde ein HLA-DR4Dw4(DRB1*0401)-Bindemotiv in der Region 102 - 110 im Hev b 1 vorhergesagt. Die Region 102 - 110 stellt eine Teilsequenz des experimentell ermittelten T-Zellepitops 91 - 109 dar. Durch Bindungsstudien der Hev b 1-Peptide an das isolierte HLA-DRB1*0401 konnte gezeigt werden, dass das Peptid 91 - 109 zu den drei Hev b 1-Peptiden gehört, die sehr gut an HLA-DRB1*0401 binden. Die Vorinkubation mit anti-HLA-DR inhibierte in PBMC von neun der zehn Patienten die rHev b 1-induzierte Proliferation. Anti-HLA-DQ bzw. -HLA-DP-Vorinkubationen führten ebenfalls zu einer Inhibition der Proliferationsantwort, so dass für die Antigenpräsentation der meisten untersuchten Latexallergiker auch HLA-DQ bzw. -DP von Bedeutung sind. Die Untersuchungen belegten, dass das Hev b 1-Peptid 91 - 109 ein T-Zellepitop mit guter Bindung an isoliertes HLA-DRB1*0401 ist. Da die Hev b 1-Antwort auch HLA-DR abhängig ist, spielt dieses Epitop bei der Allergieauslösung möglicherweise eine entscheidende Rolle. Für zukünftige immuntherapeutische Ansätze könnten diese Befunde von praktischer Bedeutung sein.
