Title: Ischemic-microvascular ocular motor nerve palsies: a 7-year experience of the Neuro-Ophthalmology Unit, Caracas Vargas Hospital
Abstract: This is a descriptive and retrospective study aimed at establishing the clinical and epidemiological characteristics of ischemic oculomotor nerves palsies (IOMNP) in Venezuela, and also to compare our data with that from other series. One hundred and eleven consecutive patients from the Neuro-ophthalmology Unit outpatient clinic of the Hospital Vargas and one of the author's private practice (MRG) were considered in this study. The mean age was 61.5 years. There was no gender preference. Orbital pain was present in one third of cases. The most frequently affected cranial nerve was the sixth, followed by the third and lastly the fourth cranial nerve. Risk factors associated to the development of IOMNP were, age older 50 years (86%), diabetes mellitus (45%), arterial hypertension (43%), hyperlipidemia (30%) and smoking (29%). Recurrence was more frequent for the third cranial nerve, followed by the sixth and fourth cranial nerves. The mean duration of the ocular motor palsy in our patients was 67 days. Fifty-eight percent of our patients did not have a referral note at the time of consultation. We describe four patients with transient oculomotor nerve aberrant regeneration after ischemic third nerve palsy. This finding has not been described previously in the literature. Photographic demonstration of one such case is presented.
