Title: Baseline and Serial Changes in Urinary Albumin-to-creatinine Ratio in Patients with Hypertension Treated by Telmisartan
Abstract: Objective: The prognostic value of microalbuminuria is demonstrated in diabetic and heart failure patients and it is reported that treatment with telmisartan, an angiotensin II receptor blocker, reduced the microalbuminuria in Japanese type 2 diabetic patients. However, it has not been reported whether microalbuminuria is seen in hypertensive patients or not. So we measured prospectively urinary albumin-to-creatinine ratio (UACR) in hypertensive patients, and then investigated whether telmisartan reduces the UACR in those patients. Methods: After a written informed consent was obtained, we measured prospectively UACR in 13 hypertensive patients (7 males, 54%) at baseline. After treating with telmisartan, 20 to 40mg per day, we measured UACR at the 6 months follow-up. Results: Reduction of systolic and diastolic blood pressure was seen at the 6 months follow-up compared to baseline (131.2 ± 12.8 vs. 146.8 ± 8.5 (P=0.003), 77.1 ± 7.2 vs. 85.8 ± 10.4 (P=0.044), respectively). Reduction of UACR was also seen at the 6 months follow-up compared to baseline (14.3 ± 16.5 (ranged from 2.1 to 81,1) vs. 26.6 ± 26.9, (ranged from 1.2 to 55,1) to, P=0.048). Conclusions: The present study demonstrates that the reduction of UACR was shown at the 6 months follow-up in hypertensive patients by treating with telmisartan. The effects of telmisartan on UACR support its use in patients with hypertension in early stage.
