Title: Comparison of Computed Tomographic Angiography and Digital Subtraction Angiography in the Initial Management of Patients with Subarachnoid Hemorrhage
Abstract: Advances in computed tomographic angiography (CTA) have been rapid. Increasing numbers of centers are replacing digital subtraction angiography (DSA) with CTA in the assessment of patients with subarachnoid hemorrhage (SAH). Advantages include the speed of image acquisition and the noninvasive nature of the test. However, the efficacy of CTA use alone has not been established. We compared the results of simultaneously performed CTA and DSA in patients with SAH. We conducted a retrospective review of patients presenting to a single institution between 2003 and 2005 with SAH who received both CTA and DSA on arrival for lesion detection. Dictated reports of each patient's CTA and DSA were compared and differences in reported lesions and/or lesion characteristics were noted. In addition, the CTA and DSA images were reviewed by two neurosurgeons. Patients were categorized according to whether or not DSA added value to the CTA and whether or not the additional...
