Title: Plazentalokalisation und Inzidenz der Präeklampsie
Abstract: Schlulßfolgerung: Es ist bekannt, daß uteroplazentare Dopplermessungen bei lateralem Plazentasitz signifi-kante Seitendifferenzen aufweisen, und diese wiederum scheinen mit einer ungünstigen Prognose insbesondere infolge Präeklampsie belastet zu sein. Deshalb haben wir die Beziehung zwischen sonographischem Plazentasitz und späterer Manifestation einer Präeklampsie prospektiv untersucht. Methode: 184 Patientinnen mit einem Gestationsalter zwischen 24 und 36 Wochen wurden prospektiv untersucht und die Plazentalokalisation sonographisch als zentral oder lateral klassifiziert. Die Präeklampsie war definiert als Kombination aus Hypertonie =140/90 mit Proteinurie =500mg/24h. Ergebnisse: Nach Berücksichtigung von Ausschlußkriterien konnten 148 Patientinnen ausgewertet werden; 115 hatten einen lateralen und 33 einen zentralen Plazentasitz. Die Inzidenz der Präeklampsie war bei lateralem Plazentasitz mit 32/115 signifikant höher als bei zentralem Sitz mit 3/33 (28% versus 9%). Schlußfolgerung: Bei lateralem Plazentasitz ist die Inzidenz der Präeklampsie signifikant erhöht mit einem relativen Risiko von 3,1 gegenüber dem zentralen Plazentasitz.
