Title: Pigmented Villonodular Synovitis of the Temporomandibular Joint Presenting as a Middle Cranial Fossa Tumor
Abstract: Pigmented villonodular synovitis (PVNS) is an infrequently occurring proliferative lesion arising from the synovial membranes of particular body joints, bursae, and associated tendon sheaths. 1 Jaffe H.L. Both nodular and diffuse forms of PVNS are currently recognized. 4 Cascone P. Rinna C. Ungari C. et al. Pigmented villonodular synovitis of the temporomandibular joint. J Craniofac Surg. 2005; 16: 712 Crossref PubMed Scopus (19) Google Scholar The most common nodular patterns associated with PVNS include giant cell tumor, xanthoma, xanthogranuloma, and myeloplaxoma, with each form generally tending to affect a discrete part of the synovium. 4 Cascone P. Rinna C. Ungari C. et al. Pigmented villonodular synovitis of the temporomandibular joint. J Craniofac Surg. 2005; 16: 712 Crossref PubMed Scopus (19) Google Scholar Diffuse PVNS, which typically affects the entire synovium, has been referred to as giant cell fibrohemangioma, chronic hemorrhagic villous synovitis, and benign polymorphocellular tumor. 4 Cascone P. Rinna C. Ungari C. et al. Pigmented villonodular synovitis of the temporomandibular joint. J Craniofac Surg. 2005; 16: 712 Crossref PubMed Scopus (19) Google Scholar Although PVNS can occur in virtually any joint of the human body, approximately 80% of cases involve the knee, with the hip, ankle, foot, hand, elbow, and shoulder accounting for almost all other cases. To our knowledge, PVNS involvement of the temporomandibular joint (TMJ) is very uncommon, with only 45 cases reported in the literature since 1973 (Table 1).
