Title: Neurophysiological studies of peripheral nerve functions after neural regeneration following nerve suture in man
Abstract: SummaryEleven patients, of which the majority had sustained total transection of the median nerve at the wrist, were investigated at least 5 years after epineural nerve suture. Electrophysiological examinations included conventional electromyography and electroneurography, as well as percutaneous microelectrode exploration of the sutured nerves proximal to the site of injury. In all cases the functional neural recovery was incomplete as evaluated by electrophysiological techniques. The most satisfactory clinical and electrophysiological recovery was observed in the youngest patients with the shortest delay from injury to nerve suture. Aside from reduced conduction velocity single regenerated myelinated fibres had qualitatively normal response characteristics and receptive fields as revealed by recording with microelectrodes. Multi-unit recordings however disclosed abnormal fascicular receptive fields which consisted of two to four separate skin patches in reinnervated skin. This misdirection of regenerated nerve fibres may account for faulty sensory localization of light tactile stimuli in the area of reinnervation. The clinical implications of the appearance of this phenomenon during the process of reinnervation are discussed.RésuméOnze patients, dont la majorité avait eu le nerf median sectionné transversalement au niveau du poignet, ont fait l'objet d'examens au moins cinq ans après la suture épineurale du nerf. Les examens électrophysiologiques comprenaient l'électromyographie et l'électroneurographie conventionnelles, ainsi que l'exploration des nerfs suturés de la région de la blessure par microélectrodes percutanés. Dans tous les cas le rétablissement fonctionnel du nerf n'était que partiel comme ont pu le déterminer les techniques électrophysiologiques employées. Le rétablissement le plus satisfaisant au point de vue clinique et électrophysiologique a pu ětre observé chez les patients les plus jeunes, sur lesquels la suture du nerf avait été effectuée dans les plus brefs délais. Malgré une reduction de la vitesse de conduction, les fibres simples myélinisées et régénérées avaient des caractéristiques de réponse et des champs réceptifs normaux au point de vue qualitatif, comme l'ont révélé les enregistrements effectués par microélectrodes. Toutefois, des enregistrements multi-unitaires ont indiqué des champs réceptifs fasciculaires anormaux qui consistaient en deux à quatre plaques de peau séparées dans la peau réinnervée. Cette mauvaise orientation des fibres des nerfs regénérés pourrait ětre la cause de la localisation sensorielle incorrecte de légers stimuli tactiles dans la zone de réinnervation. In allen Fällen war die funktionelle Nervenwiederbelebung unvollständig, wie die elektrophysiologische Technik zeigte. Die erfolgreichste klinische und elektrophysiologische Wiederherstellung fand man bei den jüngsten Patienten mit der kürzesten Zwischenzeit von Verletzung bis zur Nervennaht. Abgesehen von reduzierter Leitungsgeschwindigkeit hatten einzelne regenerierte myelinierte Fasern qualitativ normale Antwortscharakteristika und sensible Bereiche, wie Aufnahmen mit Mikroelektroden zeigten. Multieinheitsaufnahmen jedoch offenbarten abnormale fazikuläre sensible Felder, welche aus zwei bis vier separaten Hautkomplexen in der reinnervierten Haut bestanden. Diese Fehlleitung regenerierter Nervenfasern mag daran Schuld tragen, dass eine falsche Gefühlseslokalisierung leichter Taststimuli im Bereich der Reinnervierung erfolgt. Die klinischen Folgerungen aus diesen Erscheinungen im Zusammenhng mit dem. Prozess der Reinnervierung werden besprochen.
