Title: Comparison of Twice and Four Times a Day Amoxicillin with Proton-Pump Inhibitor, Clarithromycin for Helicobacter Pylori Infection: A Randomized Study
Abstract: Background: A proton pump inhibitor (PPI)-based triple therapy with clarithromycin and amoxicillin is now a standard regimen for Helicobacter pylori (H. pylori) eradication therapy in Korea. Amoxicillin has time-dependent bactericidal activity for against H. pylori. Aim: To evaluate and compare the efficacy of two amoxicillin regimens for treatment of H. pylori in a randomized study. Methods: One hundred twenty-one patients with H. pylori infection confirmed by endoscopy and histology were eligible to this study. H. pylori infection was proven by histology or rapid urease test. Patients randomly assigned to one of the two regimens: amoxicillin 1000 mg bid, clarithromycin 500 mg bid, and omeprazol 20 mg bid for 2 weeks (Group A, N = 58), or amoxicillin 500 mg qid, clarithromycin 500 mg bid, and omeprazol 20mg bid for 2 weeks (Group B, n = 63). The eradication rates of H. pylori were evaluated by 13C-urea-breath test 4 weeks after completion of treatment.Results: 120 patients completed the trial (57 Group A, 63 Group B). The eradication rates were 89.5% in Group A, 88.9% in Group B (P > 0.05). Compliances in two groups were not different. Side effects in two groups were generally mild and only one patient of Group A discontinued treatment due to adverse effects.Conclusion: Both the twice and the four times daily amoxicillin regimens were equally effective and safe for H. pylori eradication therapies. Therefore, the twice a day amoxicillin regimen was preferred for the patients comfort.
