Title: Malignome der inneren Nase und der Nasennebenhöhlen, Klinik, Therapie und Prognose verschiedener Tumorentitäten
Abstract: Hintergrund: Malignome der Nasenhaupthöhle (NHH) und der Nasennebenhöhlen (NNH) sind seltene Tumorerkrankungen. Sie machen etwa 3 % der Malignome im Kopf-Hals-Bereich aus. Patienten und Methode: In den Jahren 1998 bis 2002 wurde an der HNO-Klinik der Ruhr-Universität Bochum bei insgesamt 46 Patienten die Erstdiagnose eines Malignoms der NHH bzw. NNH gestellt. Ergebnisse: 18 der 46 Malignome waren Plattenepithelkarzinome. Bei der Erstdiagnose lagen 52 % der Malignome im T4-Stadium vor. In 17 % der Fälle konnte der Ausgangsort des Tumors nicht mehr identifiziert werden. Weitere 43 % der Malignome nahmen ihren Ausgang von der NHH, 20 % von der Kieferhöhle. Bei 11 der 46 Patienten (24 %) lagen bei Erstdiagnose bereits zervikale Lymphknotenfiliae vor. 92 % der Patienten wurden primär operiert. 25 Patienten (54 %) wurden zusätzlich bestrahlt. Die Einjahres-Überlebensrate des Gesamtkollektivs betrug 75 %, die Zweijahres-Überlebensrate 47 %. Lediglich der operativ erzielte Resektionsgrad, der Lymphknotenstatus bei Erstdiagnose und das Patientenalter konnten als unabhängige Faktoren für das Überleben identifiziert werden. 76 % der Patienten (16/21) verstarben an einem Lokalrezidiv. Rezidive traten bei Malignomen der Kieferhöhle häufiger auf als bei Malignomen der NHH und der Siebbeinzellen. Schlussfolgerungen: Malignome der NHH und der NNH werden oft erst in fortgeschrittenen Tumorstadien diagnostiziert. Das Rezidiv-Verhalten bestimmt die limitierte Prognose. Prospektive, multizentrische Studien zur Therapie der Malignome sind dringend erforderlich.
