Abstract: _Eine optimale Symptomkontrolle und die Verhinderung der Krankheitsprogression gehoren zu den Hauptzielen des COPD-Managements, betonte Prof. Tobias Welte, Hannover. Um diese zu erreichen, empehlt die Global Initiative for Chronic Obstructive Lung Disease (GOLD) im Rahmen einer medikamentosen COPD-erapie die kontinuierliche Behandlung mit langwirksamen Bronchodilatatoren. In einschlagigen Studien erwies sich die Fixkombination aus dem langwirksamen β-Agonisten (LABA) Indacaterol und dem langwirksamen Muskarin-Antagonisten (LAMA) Glycopyrronium gegenuber den Monotherapien mit kurzwirksamen Anticholinergika, β2Mimetika oder inhalativen Kortikosteroiden als uberlegen. Diese Wirkstoombination steht unter der Bezeichnung Ultibro® Breezhaler® (QVA149) zur Verfugung. Im Rahmen der QUANTIFY-Studie wurde der Eiekt der Fixkombination Indacaterol/Glycopyrronium mit dem der Einzelsubstanzen Tiotropium und Formoterol verglichen. Hierbei erwies sich der duale Bronchodilatator gegenuber den Einzelsubstanzen als nicht-unterlegen, so das Studienergebnis. Zudem verbesserte sich das Symptom „Atemnot“ signikant unter der Fixkombination gegenuber der Vergleichsmedikation. Dieser Eiekt erklart sich aus der synergistischen Wirkung des β2-Agonisten und Muskarin-Antagonisten am Zielort, kommentierte Welte. ■
