Title: Diffusion Tensor Imaging of Cerebral Peduncle in Patients with Acute Striatocapsular Infarction Complicated by Upper Limb Motor Functional Damage
Abstract: Objective Using diffusion tensor imaging( DTI) to study the diffusion changes in cerebral peduncle in patients with acute striatocapsular infarction complicated by upper limb motor functional damage,and to discuss the relationship between the striatocapsular infarction and the upper limb motor functional damage. Methods A total of 18 patients with unilateral upper limb motor functional damage after the first onset of acute striatocapsular infarction were enrolled in this study. DTI scanning was performed in all patients within two weeks after the onset of the disease. The fractional anisotropy( FA) and the mean diffusivity( MD) of bilateral cerebral peduncles were measured,and the asymmetry index( AI)of bilateral FA was calculated. The correlation between AI and the involved upper limb motor function scores was evaluated. Results Two weeks after the onset of the disease,the FA value of cerebral peduncle at affected side was significantly lower than that at the unaffected side,and no significant difference in the MD values existed between the two side. AI carried a statistically significant negative relationship with the involved upper limb motor function scores( r =- 0. 91,P 0.01). Conclusion DTI can detect the early diffusion changes in cerebral peduncle after striatocapsular infarction. The integrity of pyramidal tract in cerebral peduncle is closely correlated with the motor function of involved upper limb. DTI can provide quantitative imaging reference for clinical assessment of striatocapsular infarction.
