Title: Amiodarone combined with losartan for preventing recurrence of atrial fibrillation after cardioversion
Abstract: Objective To observe the effect of amiodarone combined with losartan for maintaining sinus rhythm of in the treatment of paroxysmal or persistent atrial fibrillation after cardioversion.Method The patients(n=80) with paroxysmal or persistent atrial fibrillation with conversion indicants of sinus rhythm were randomly divided into two groups, including amiodarone group(n=40) and amiodarone combined with losartan group(n=40).The treatment with amiodarone was stopped after 12 months, and the patients were followed up for 18 months. The influences of amiodarone and losartan on the maintenance rate of sinus rhythm and atrial remodeling were observed.Result On the 12th and 18th follow-up month, the maintenance rate of sinus rhythm was 45.9% and 35.1% respectively in amiodarone group, and 79.5% and 59.0% in amiodarone combined with losartan group.Amiodarone group combined with losartan had a higher maintenance rate of sinus rhythm significantly than amiodarone group(average P0.05).The patients with permanent atrial fibrillation in amiodarone combined with losartan group(n=8) were much less than those in amiodarone group(n=13,P0.05).The left atrial dimension(LAD)of the patient changed significantly in amiodarone group before and after the treatment[before treatment(37.7±6.2) mm,18 months after treatment(39.2±6.5) mm,P0.05],while that did not change significantly in amiodarone combined with losartan group[before treatment(38.1±5.8) mm,18 months after treatment(38.4±6.1) mm,P0.05].Conclusion Amiodarone combined with losartan can improve the effect of maintaining sinus rhythm of atrial fibrillation after cardioversion, and prevent atrial remodeling.
