Title: Anatomic evaluation of exposure and damage to cerebellopontine angle region via enlarged middle cranial fossa approach
Abstract: Objective To evaluate comprehensively the exposure and damage to the cerebellopontine angle region by the middle cranial fossa approach.Methods The study was conducted in 6 Chinese adult cadaveric heads(12 sides) by mimicking the operation via middle fossa approach.Extended resections of the superior wall of internal acoustic meatus and the arcuate eminence of temporal bone were performed after the middle cranial fossa approach was finished.The exposure degree of each step was evaluated according to Ammirat surgical exposure grading system.Horgan method was used for calculating the exposure area.Results The scores were 0,2,3 and the exposure areas for each of the three steps were 0 mm2,(523.00±101.98) mm2,(706.30±119.39) mm2 respectively,and the damage degrees were 1,5 and 10.Conclusion Simple middle cranial fossa approach can not expose the cerebellopotine angle region sufficiently.In company with the increase of the exposure degree and area of each step of the middle cranial fossa approach,the damage degree increases,especially after the resection of arcuate eminence of temporal bone.
