Title: PRESUMPTIVE THERAPY IN STREPTOCOCCAL PHARYNGITIS
Abstract: QA 6-year-old child was recently seen in the office with fever, sore throat, enlarged and tender tonsillar lymph nodes and beefy red tonsils without exudate. The throat culture yielded heavy growth of Group A streptococci and penicillin was started. Two days later two siblings developed low grade fever and began to complain of sore throat. Is examination and culture of the siblings necessary or can treatment be started solely on the basis of a history of exposure? The latter would save the family considerable expense. Would the approach differ if the index case was diagnosed on the basis of a rapid antigen detection test for Group A streptococci?
